Substitute Forms

Paperwork Required for Payroll Purposes
As a Tri-County Substitute, you are eligible to substitute for Colusa Unified School District.  However, it is necessary for all employing agencies who participate in the Consortium to have original payroll forms with wet signatures on file rather than copies.  This is discussed with you at the time of your orientation for the Tri-County Substitute Teacher Consortium held at the Sutter County Superintendent of Schools office.
Since the Tri-County Substitute Teacher Consortium is no longer able to share payroll information with this office, it is required that you complete the following documents and return them to our District Office within 30 days of receipt of this email.  Please note that incomplete information or failure to submit the documents will delay the processing of future payments to you.
Forms include the following:
  • Federal Tax Form W-4 - (1st page only)
  • U.S. Citizenship and Immigration Services Form I-9
  • CalSTRS Permissive Membership Form ES 0350 - (If you have not already elected membership and wish to)
Forms with original signatures must be mailed or hand-delivered to the following address:
